Everything You Need to Credential and Launch a Healthcare Practice Successfully

Starting a new practice involves many moving parts. This checklist focuses on the credentialing and enrollment steps you need to complete to become fully operational and bill insurance.

 Step 1: Set Up Legal & Business Structure

  • ☐ Register your business (LLC, PLLC, or Corporation)
     
  • ☐ Obtain EIN (Employer Identification Number) from IRS
     
  • ☐ Apply for state business licenses (if required)
     
  • ☐ Open a business bank account
     

Step 2: Obtain Core Identifiers

  • ☐ Apply for individual NPI (Type 1) for each provider
     
  • ☐ Apply for group NPI (Type 2) for the practice
     
  • ☐ Register for a Taxonomy Code (matching specialty)
     
  • ☐ Obtain a DEA certificate (and state CDS license, if applicable)
